Physician vs. Midwife at Homebirths

Abstract

The effect of attending breech, twin, and post-date pregnancies on home birth outcomes was assessed. The same from was used to collect data on a convenience sample of 4,361 home births attended by apprentice-trained midwives from 1970 to 1985 and 4,107 home births attended by family physicians from 1969 to 1981. Data sets were compared to find 1,000 pairs of pregnant women, one from each group, who were matched for age, sex, socioeconomic status, race, and medical risk. The perinatal mortality rate for the midwife-attended births was 14 per 1,000 (three fetal deaths before labor, six intrapartum fetal deaths, and five neonatal deaths). The perinatal mortality rate for births attended by family physicians was five per 1,000 (one fetal death before labor, two intrapartum fetal deaths, and two neonatal deaths). The difference was statistically significant; however, the differences disappeared when cases involving post-dates, twin, or breech deliveries were eliminated from the sample. Although the data are more than a decade old, they support the premise that outcomes for low-risk home births are comparably good whether attended by physicians or midwives. However, the findings do raise questions about the safety of attending high-risk births at home.

Breech Twin Birth

I posted this one on the FB business page, but I will add the link here.  It's a mother who makes a choice to move and stay with family in another state, to be able to give birth to her vaginal twin breech babies.  The pictures are incredible, as is the mommy!

A VBA3C: Story

The link below is from the Birth Without Fear Blog.  It was written in January.  It is an amazing story of a mom who decided she was going to have a vaginal birth.  She had the support of her physician, doula and family to get her through an incredibly long and intense process.  It's definitely worth the read.

Welcome Baby Anders - Born in the Caul

Please welcome Baby Anders! He was born on Tuesday, February 26, 2013 at 9:33pm. He weighed in at 7.8 lbs and was 19-3/4 inches long.   This little man was born via water birth and was born with his amniotic sac still intact.  The term for that is "born in the caul".  It is not very often that babes are born in the caul, so most cultures consider it sacred, extra special and a huge blessing!  Some cultures believed that a baby born in the caul was destine for greatness.  We already knew Anders was going to be extra special!!

His mother presented having thought her membranes may have ruptured, but consistent labor had not set in.  We all helped to urge things along.  It wasn't until that afternoon that it was discovered her membranes were indeed still intact.  About that time real labor decided to set in.

Megan walked a lot, and pushed fluids and snacked.  She continued to squat and stretch throughout the process.  She utilized acupuncture to help facilitate labor.  Dr. Stephanie Hoglund was kind enough to come over and assist with the acupuncture as well as hip adjustments and massage.  It was a beautiful day for a little one to be born and both mom and dad opted to walk in town and browsing some local stores.  Eventually Megan settled in by sitting on a birth ball and opted to watch Pride & Prejudice, munch on a Subway sandwich and work her way through her contractions.  Megan is a well-educated mother and her husband, Ethan, has spent as much time studying on parenting and how to help his wife through labor.  Ethan was a HUGE support loving and nurturing Megan through her entire laboring process.  He was also completely involved in Baby Anders care right after birth.

A New Anti-Breech Hospital Policy

Care to weigh in on this one?  A hospital in Hudson, WI has now created a policy that all breech will be delivered via cesarean section.  On top of that, you will no longer be allowed to decline your right as a parent to not have a cesarean section.  Stand and Deliver is blogging about it.  You can follow the information at her blog
